Wellness Program

The physicians at FORM orthopaedics are pleased to bring a comprehensive wellness program to the Tri-Valley Area.

What is a wellness program?

A wellness program can be defined in many ways; a healthy eating program or an exercise program; a weight-loss program; or a program that a physician puts together to reduce high blood pressure or improve mobility and strength in an elderly individual. A “Wellness Program” is really a plan to help individuals’ live healthy and active lifestyles.

The FORM Wellness Program consists of four key components:

Physician Evaluation and Medical Clearance

Prior to beginning any physical fitness program it is essential to have a complete physical exam. During this exam, patients will be evaluated and goals will be discussed for both short and long term success. The evaluation is especially important for those individuals with certain risk factors such as, sedentary lifestyle, smoking, family history of heart disease, obesity and hypertension.

Dr. Jim Dhanoa, board certified in Family Medicine with added certification in Sports Medicine, will conduct all exams and personally discuss each patient’s history and future wellness goals.

Physical Fitness and Therapy Program

Once a patient has been Medically Cleared, he or she will be evaluated by a licensed physical therapist to develop a safe, fun exercise regimen with the patient’s goals in mind. For patient convenience, Implementation of this program will take place at the Farwell Medical Building, the same location of the initial Physical Evaluation. Supervision, progress assessments, and program progression is all part of the program for 12 visits.

Consultation with a Nutritionist

During the program patients will work with a nutritionist to develop a healthy eating program to match the goals that have been set forth. Proper diet is essential for living well and maintaining good health. Weight loss, management of diabetes, high cholesterol and high blood pressure are just a few things that our nutritionist can address with our patients.

Transition to an Independent Program

Once the formal wellness program is complete, our physicians and staff will help to transition each patient to an independent program. The independent program will be based on each individual’s lifestyle and goals for continued health and wellness.
